Varje år sköter Karolinska Universitetssjukhuset cirka 1,6 miljoner patientbesök och det är mycket viktig att patienten får rätt vård. Men tyvärr enligt Karolinska Universitetssjukhuset kvalitetsbokslut 2012, påvisas det att nästan var tredje ordination är felaktig, allt från preparat och styrka till dosering. För hantering av läkemedel använder sig Karolinska Universitetssjukhuset av det digitala patientjournalsystemet Take Care i nuvarande läge. För att förhindra att det blir fler felaktiga ordinationer, har detta examensarbete undersökt de olika problemen utifrån slutanvändarna, alltså läkarna och sjuksköterskorna som har med läkemedelssystem som de använder idag att göra. För att ta reda på de olika problemen, gjordes ett antal olika undersökningar, bland annat fältstudier, intervjuer, en enkät och genomgång av avvikelserapporter.

När undersökningarna hade genomförts, föreslogs olika designlösningar som fungerar som rekommendationer av vad systemet bör ha med hänsyn till användbarheten, och samtidigt ge Karolinska Universitets sjukhus en viss kompetens om vad som behövs begära från leverantören. En av slutsatserna som konstaterades var att en större förändring utifrån andra aspekter så som utbildning, erfarenhet, tidsbrist, stress och rutiner behöver också tas hänsyn, då det inte handlar bara att designa rätt utan att det som händer runt omkring systemet ska också fungera rätt.

Each year Karolinska University Hospital handles approximately 1.6 million patients and it is of up most importance that each patient requires the appropriate care. But unfortunately, according to Karolinska University Hospital quality financial statements in 2012, showed that almost every third prescription is inaccurate, ranging from medicine to dosages.

For the management of medications records uses Karolinska University Hospital the electronic medical record system Take Care as the current.

To prevent additional inaccurate prescriptions, this thesis investigated the various problems based on end users, such as doctors and nurses who regularly use the medical record system.

To find out the various problems, a number of researches were executed, including field studies, interviews, a survey and review of incident reports.

 

Once the studies were conducted, various design proposals were recommended and served as recommendations of what the system should require in terms of usability, while providing Karolinska Unversity Hospital some kind of expertise on what is needed to request from the supplier. One of the conclusions that were found, were that a major change based on other aspects such as education, experience, lack of time, stress and routines also need to be taken into account, because it is not just about correct design, but also that the presented aspects outside of the system work coherently.
