    Abstract [en]

    The increased focus on patient safety in hospitals has yielded a flood of new technologies and tools seeking to improve the quality of patient care at the point-of-care. Hospitals are complex institutions by nature, and are constantly challenged to improve the quality of healthcare delivered to patients while trying to reduce the rate of medical errors and improve patient safety. Here a simple mistake such as patient misidentification, specimen misidentification, wrong medication, or wrong blood transfusion can cause the loss of a patient's life. The focus of this paper is the implementation and evaluation of a handheld-based patient identification system that uses radio frequency identification (RFID) and 802.11b wireless networks to identify patients. In this approach, each patient is given a RFID wristband which contains demographic information (patient ID number, patient summary, hospital code) of the patient. A handheld device equipped with 802.11b wireless connectivity and a RFID reader is then used by the medical staff to read the patient's wristband and identify the patient. This work was carried out at the Department of Medical Physics and Bioengineering at the University College Hospital Galway, Ireland and in co-operation with the National University of Ireland, Galway.

    Finding a cost-optimal preventive maintenance interval: A study on ECG devices in Region Östergötland2017Independent thesis Advanced level (degree of Master (Two Years)), 20 credits / 30 HE creditsStudent thesis
    Abstract [en]

    Medical equipment maintenance is costly and the question has been raised whether the amount of preventive maintenance (PM) today is effective from a cost perspective. The goal was therefore to find the cost-optimal interval on which PM should be performed. By analysing data on previous maintenance actions and failures of ECG devices in Region Östergötland, a model describing the relation between preventive maintenance interval length and number of failures was found. Together with average costs of maintenance actions, this was used to calculate the total maintenance costs for different preventive maintenance intervals. The optimal interval was found to be 450 days on a 10 year perspective, but decreasing for longer perspectives. Even though the result is specific for ECG in Region Östergötland, the methodology, with some adjustments and improvements, could be used for other devices to decide the optimal maintenance interval and for example also to evaluate when to invest in new devices.

    Abstract [en]

    Purpose: The purpose of this study was to analyze a database of completed International Outcome Inventory for Hearing Aids (IOI-HA) questionnaires obtained from over 100,000 clients fitted with new hearing aids in Sweden during the period of 2012-2016. Mean IOI-HA total scores were correlated with degree of hearing loss, unilateral versus bilateral fitting, first-time versus return clients, gender, and variation among dispensing clinics. The correlations with expectations, service quality, and technical functioning of the hearing aids were also analyzed. Method: Questionnaires containing the 7 IOI-HA items as well as questions concerning some additional issues were mailed to clients 3-6 months after fitting of new hearing aids. The questionnaires were returned to and analyzed by an independent research institute. Results: More than 100 dispensing clinics nationwide take part in this project. A response rate of 52.6% resulted in 106,631 data sets after excluding incomplete questionnaires. Forty-six percent of the responders were women, and 54% were men. The largest difference in mean score (0.66) was found for the IOI-HA item "use" between return clients and first-time users. Women reported significantly higher (better) scores for the item "impact on others" compared with men. The bilaterally fitted subgroup reported significantly higher scores for all 7 items compared with the unilaterally fitted subgroup. Experienced users produced higher scores on benefit and satisfaction items, whereas first-time users gave higher scores for residual problems. No correlation was found between mean IOI-HA total score and average hearing threshold level (pure-tone average [ PTA]). Mean IOI-HA total scores were found to correlate significantly with perceived service quality of the dispensing center and with the technical functionality of the hearing aids. Conclusions: When comparing mean IOI-HA total scores from different studies or between groups, differences with regard to hearing aid experience, gender, and unilateral versus bilateral fitting have to be considered. No correlation was found between mean IOI-HA total score and degree of hearing loss in terms of PTA. Thus, PTA is not a reliable predictor of benefit and satisfaction of hearing aid provision as represented by the IOI-HA items. Identification of a specific lower fence in PTA for hearing aid candidacy is therefore to be avoided. Large differences were found in mean IOI-HA total scores related to different dispensing centers.

    Abstract [en]

    Low levels of physical activity and sedentary behaviour are a growing health problem globally. Physical inactivity is associated with increased risk of numerous ailments, cardiovascular disease and mortality. Our primary aim was to perform a feasibility study on how to incorporate physical activity among students and teachers in regular teaching activities. The second aim was to investigate how students and teachers perceived the differences between outdoor walking seminars and regular indoor seminars. By transforming an on-campus course into a blended course, we were able to conduct seminars outdoors in nearby nature while walking. These walking seminars were evaluated among 131 students and nine teachers leading the walking seminars. The responses to the student survey and teacher interviews indicate that discussions, sense of well-being and the general quality of the seminar improved, regardless of how physically active participants were the rest of the time. The study shows one way to increase physical activity with small means; in our case, a reorganization of how we prepared for the seminars which allowed for walking discussions.

    How user characteristics affect use patterns in web-based illness management support for patients with breast and prostate cancer.2013In: Journal of medical Internet research, ISSN 1438-8871, Vol. 15, no 3, p. e34-Article in journal (Refereed)

    BACKGROUND: Frequently eHealth applications are not used as intended and they have high attrition rates; therefore, a better understanding of patients' need for support is warranted. Specifically, more research is needed to identify which system components target different patient groups and under what conditions.

    OBJECTIVE: To explore user characteristics associated with the use of different system components of a Web-based illness management support system for cancer patients (WebChoice).

    METHODS: For this secondary post hoc analysis of a large randomized controlled trial (RCT), in which WebChoice was tested among 325 breast cancer and prostate cancer patients who were followed with repeated measures for 1 year, usage patterns of 162 cancer patients in the intervention arm with access to WebChoice were extracted from the user log. Logistic regression was performed to identify patterns of associations between system use and patient characteristics. Latent class analyses (LCA) were performed to identify associations among the use of different system components and levels of social support, symptom distress, depression, self-efficacy, and health-related quality of life.

    RESULTS: Approximately two-thirds (103/162, 63.6%) of the patients logged on to WebChoice more than once, and were defined as users. A high level of computer experience (odds ratio [OR] 3.77, 95% CI 1.20-11.91) and not having other illnesses in addition to cancer (OR 2.10, 95% CI 1.02-4.34) increased the overall probability of using WebChoice. LCA showed that both men with prostate cancer and women with breast cancer who had low scores on social support accompanied with high levels of symptom distress and high levels of depression were more likely to use the e-message component. For men with prostate cancer, these variables were also associated with high use of the self-management advice component. We found important differences between men with prostate cancer and women with breast cancer when associations between WebChoice use and each user characteristic were analyzed separately. High use of all components was associated with low levels of social support among women with breast cancer, but not among men with prostate cancer. High use of e-messages, advice, and the discussion forum were associated with high levels of depression among women with breast cancer, but not among men with prostate cancer. For men with prostate cancer (but not women with breast cancer), high use of symptom assessments, advice, and the discussion forum were associated with high levels of symptom distress. However, it is unclear whether these findings can be attributed to differences related to diagnosis, gender, or both.

    CONCLUSIONS: This study provides evidence that different user characteristics are associated with different use patterns. Such information is crucial to target Web-based support systems to different patient groups. LCA is a useful technique to identify subgroups of users. In our study, e-messages and self-management advice were highly used components for patients who had low levels of social support and high illness burden, suggesting that patients with these characteristics may find such tools particularly useful.

    Abstract [en]

    Work-related musculoskeletal disorders are associated with high costs and it is essential to prevent them. Occupational Health Services(OHS) provide expert services regarding work environment, health and rehabilitation of work related disorders. Risk assessments of the work environment can be an initial step for preventive measures, and ergonomists can be assigned by clients to assess exposures in the work environment. For such assignments different tools can be used. The aim of this thesis was to explore prerequisites, processes and practices of OHS consultants in Sweden within the domain of primary prevention of work-related musculoskeletal disorders. This was explored through the work of ergonomists in their role of assessing ergonomics risks. The research methodology was both quantitative and qualitative. Data collection includes a web questionnaire, semi-structured interviews and inter- and intra-observer reliability tests. The results shows that ergonomics risk assessments were most commonly initiated reactively and a systematic work methodology for the risk assessment process was often lacking. Swedish ergonomists used only a few standardized tools for risk assessment. The Ergonomics provision from the Swedish Work Environment Authority, AFS-98, was widely used, but other observation-based tools were used far less often. Ergonomics risks were often assessed solely by means of observation, based on ergonomists’ knowledge and experience. The results also pointed to that that the reliability was not acceptable when risk assessment was performed without any standardized tool. Furthermore, the results point to that support from the OHS organizations is an important prerequisite for ergonomists to work with primary prevention. Further, opportunities for specialization within a specific industry sector seem to facilitate ergonomic interventions. It is also important to have close relationships with clients and to make them aware about ergonomists competence. Conclusively, this thesis identifies a numbers of areas in which OHS must develop to improve primary preventive services regarding work environment.

    Denna studie syftade till att undersöka hur arbetsmiljöutbildningar inom två olika områden är utformade med avseende på innehåll och omfattning. Vidare syftade studien till att undersöka vilka metoder som används för att nå ut till utbildningsmottagarna. De två områdena som undersöktes var utbildningar inom SAM och härdplastutbildningar.

    En kvalitativ intervjustudie utfördes på fyra utbildare för att undersöka deras syn på arbetsmiljöutbildningar inom SAM och hantering av risker vid hantering av produkter innehållande härdplastkomponenter. Två av utbildarna kom från företagshälsovården och två från Prevent.

    Innehållsmässigt uppfattades utbildningarna vara relativt lika, där utbildningar inom SAM fokuserade på: AML, SAM och psykosociala arbetsmiljöfaktorer. För härdplastutbildningarna uppfattades fokus ligga på vilka exponeringsrisker som finns, hur de ska undvikas och hur de ska upptäckas med hjälp av riskbedömningar. Det som varierade mellan de undersökta aktörerna var längden på utbildningarna och utbildningsmetodiken. De kortare utbildningarna var av mer föreläsande karaktär medans de längre hade mer praktiska inslag i form av riskbedömning och andra gruppövningar.

    De undersökta aktörerna uppfattades täcka in det mesta och ha kommit långt med deras utbildningskoncept. Det som uppfattas kunna påverka utbildningskvaliteten är beställarnas önskemål om att korta ner utbildningarna. Vad gäller utbildningarna inom SAM, som typiskt är inriktad till chefer, skulle dock mer fokus kunna läggas på att förmedla vikten av hur viktigt ledarskap och delaktighet är för att främja arbetstagarnas arbetsmiljö.

    The epistemological basis of clinical trials and evidence-based medicine is investigated. Clinical trials are directly action-guiding experiments on treatment effects. This is the reason why well-performed clinical trials take precedence over all other types of studies as far as treatment effects are concerned. The efficiency of public health interventions can be studied with directly action-guiding experiments that have the same strong epistemic justification as clinical trials. However, in order to assess the causality of diseases, information from several types of studies will have to be combined. Here, no single type of studies has priority over all the others. Therefore, evidence hierarchies are less helpful in studies of causality than they are in investigations of the effects of treatments or interventions.

    Background

    This project is a part of the project “Planning of new work environments - improvements of OHS competence for customer utility” which is a study conducted by the Unit of ergonomics at the KTH Royal Institute of Technology in Stockholm, Sweden. The role which ergonomists and work environment engineers play in the development of new work environments is today unclear, and an issue this study addresses.

    Aim

    The aim of this study was to investigate how involved work environment engineers and ergonomists are in planning and designing new work environments or technical equipment prior to building, rebuilding or extending premises.

    Method

    A survey was used to obtain information. An online survey was sent to ergonomists and work enviroment engineers with memberships in respective associations. The survey contained 17 questions, both multiple choice and open-ended questions, and was answered by 531 respondents.

    Result

    The most common basic educations were medical or technological. The most common employment forms were for work environment engineers private sector, occupational health services and joined occupational health services, and for ergonomists occupational health services, public employment and joined occupational health. The work environment engineers applied their knowledge primarily in industry settings, whereas the ergonomists worked with offices. The ergonomists with public employment participated least in planning and designing work environments. The work environment engineers primarily received their tasks from property developers. The tasks given to the ergonomists were given to them personally

    Conclusion

    Those involved in planning and designing of work environments were often advisors in parts of the constructions process, and did not participate throughout the entire process. Approximately 20 % of those answering the survey had never been part of such processes. Out of all repliers, only a few did not wish to participate in planning and designing of new work places.

    Abstract

    The purpose of this master thesis was to investigate the occupational health effects concerning nanoparticles of colloidal silica at a Swedish company that conducts chemical production. The study was performed at a company that manufactures a product, Product A, whose raw material, Raw material B, contains the concerned nanoparticles. The goal was to reduce health risks and to promote a good working environment.

     

    Methods used included literature studies, studies of safety data sheets, assessment of earlier measurements taken by the company, interviews and observations as well as a risk assessment method and tool.

     

    A research study about the potential health concerns regarding nanoparticles of colloidal silica indicated a cytotoxic effect on tested human alveolar epithelial cells. Although these were the findings of this study, it was also noted that too little data was present in order to be able to make any conclusions on effects of human exposure. One study showed that it was possible for nanoparticles to penetrate the skin. Other studies did not indicate adverse effects. In summary, there is too small an amount of data and knowledge gaps exist around health the effects. Therefore it is reasonable to apply the precautionary principle.

     

    Interviews with the workers and observations of the operations showed that the manufacturing process to a large extent was a closed process and that the workers’ exposure of nanoparticles mostly occurred when; transferring the product to transportation containers, cleaning, changing of filters and sampling of the product. A risk assessment of the working environment in the production of Product A was performed. Due to previous precautionary measures taken by the company it was not likely that any larger exposure was present. A number of additional measures were developed and prioritized in order to further reduce the workers’ exposure risks and promote a good working environment. The most important measure was to reduce the spillage of concentrated Product A and dilute Raw material B. Several of the measures were non-costly and quick to implement.

    Abstract [en]

    The background to this report is the author’s ambition to understand individuals’ work situation, how it’s formed in interaction between the individual and factors in the work-setting. Such an ambition is well in line with the concept of Human-Technology-Organization (HTO), a cross-scientific approach that puts a system-oriented perspective on how human, technolog-ical and organizational factors interact within work systems.

    The aim of this report was to explore managers’ work-situation and generate an understanding of it from an HTO-perspective by using an organizational model as a framework for an interview guide and to analyze the work situation from an HTO-perspective following the ques-tions at issue:

    1. How does the managers experience their work situation?

    2. How can an HTO-perspective contribute to the understanding and potential of im-provement of the work situation?

    The organizational model used in the report is Porras & Robertson’s (1992) "Factors constituting the organizational work setting". The model summarizes and describes the organiza-tional, social, physical and technological factors that up until 1992 had been identified by contemporary research within the "Organizational Development" field as important for organizational change.

    Four managers’ work situation was studied at the company Södra Cell Mörrum. The report’s methodological approach is of an explorative qualitative nature and has been conducted by the design of an interview guide based on the organizational model described above and semi structured interviews to explore the managers’ work situation.

    The results give a detailed description of how the managers experience their work situation och how it is affected by the interaction of the factors human-technlogy-organization. In analysis, it’s illustrated how:

    - the organization’s strategies and structure in interaction with the work flow and tech-nology contributes to the managers’ exposure of stress in their work-situation.

    - the organizational structure interacts with the individual and vice versa, how the managers’ fuzzy work-description contributes to how they themselves in interaction with other parts of the organization define it and creates informal organizational structure an interaction processes.

    - the use of technology is affected by its design, by how the organization reinforces its use and by individual characteristics. It has been shown how technology affects the organization’s strategical effort and disposition.

    - organizational factors such as goals and reward systems interacts with and affect behaviors at the workplace. The analysis also shows how informal aspects of the organization, such as it’s history and culture affects behaviors.

    The conclusions of the report are:

    -that an HTO-perspective is a suitable approach to create a broad understanding of the factors which interaction affects work systems. Such an understanding constitutes a good starting point for development and improvement of work systems.

    This paper presents a method based on process manager technology for making all actors (humans or information systems) involved in healthcare processes to communicate along these processes. The method utilizes straightforward and yet executable process diagrams. Furthermore, the paper suggests a number of additional features to the method that may cater for the representation of security and quality requirements, as well as enhanced efficiency of the healthcare processes.

    Design of user-centred wireless sensor technology in sports: An empirical study of elite kayak athletes2011Conference paper (Refereed)
    Abstract [en]

    University research is demanded to be more need-driven and user-centred in order to address and solve problems and needs of the market. In the present study a group of athletes and coaches has been analysed on their lead user characteristics. Some of the lead users have contributed with advanced user insights and aidin trend foresight in this sport. In a combined qualitative and quantitative approach the value of lead users in sports technology was examined and four users were identified as lead users. In consistence with previous research the results indicate that involving lead users will foster superior insight and research.

    The aim of the study was to use SAM-index data and data on factors from the report Det Goda Arbetet from the Swedish Work Environment Authority and identify if there is any relation between listed companies' market values and their systematic work environment management. This master’s work is limited to Forty-seven companies on the Swedish stock market that responded to the Work Environment Authority survey NU2012.

     

    The method in this study is a quantitative approach in which statistical, quantifiable results were sought. The companies that participated in the NU2012 survey was divided into three groups based on their results from: i) Work Environment SAM index data and ii) data on factors from the report Det Goda Arbetet.

     

    Results from SAM- index data show a relation between companies’ systematic work environment management and market capitalization for the year 2012 while the results from Det Goda Arbetet show that scores for the highest average varies between the groups. Results for the years 2008-2011 (the year when we don`t have access to the baseline study) shows that there is a tendency for a relation between share price, turnover, number of employees and work environment for these years.

     

    The main conclusion based on the analyzes of the data on the share price and the work environment in Sweden as well as the literature review included in the study, it is found that companies that have a good work Environment tend to have higher stock price and better stock market performance than companies with poor work environment management.

    Sonification designs have been increasingly used to complement data presentation and exploration in various disciplines. In medical research they have been applied as an aid for analysis and diagnosis, disease treatment, to assist visually impaired patients, and in physiotherapy. Interactive sonification as a communication channel has the potential to be used to describe health risk data to a range of audiences. The novelty of this approach, its interactive nature and ability to be combined with music may provide an effective alternative to the more dominant visual strategies (i.e. graphs, pie charts etc.), particularly when communicating with young people. In this project Using Sonification to COmmunicate public health Risk data (SCORe) we plan to test the role of sound, music and interactive sonification to communicate vital health risk information associated with alcohol consumption to young people.

    Abstract [en]

    Excessive consumption of alcohol has been recognised as a significant risk factor impacting the health of young people. Effective communication of such risk is considered to be one key step to improve behaviour. We evaluated an innovative multimedia intervention that utilised audio (sonification-using sound to display data-and music) and interactivity to support the visual communication of alcohol health risk data. A 3-arm pilot experiment was undertaken. The trial measures included health knowledge, alcohol risk perception and user experience of the intervention. Ninety-six subjects participated in the experiment. At 1 month follow-up, alcohol knowledge and alcohol risk perception improved significantly in the whole sample. However, there was no difference between the intervention groups that experienced (1) visual presentation with interactivity (VI-Exp group) and, (2) visual presentation with audio (sonification and music) and interactivity (VAI-Exp group), when compared to the control group which experienced a (3) visual only presentation (V-Cont group). Participants reported enjoying the presentations and found them educational. The majority of participants indicated that the audio, music and sonification helped to convey the information well, and, although a larger sample size is needed to fully establish the effectiveness of the different interventions, this study provides a useful model for future similar studies.

    Abstract [en]

    Online communication has become a potential means of communication between patients and health care providers, but so far few studies are published about online communication as part of nursing care. The aim of this study was to explore how an online patient-nurse communication (OPNC) service meets the information needs of men with newly diagnosed testicular cancer. We applied a qualitative approach by examining the content of online messages sent by patients to nurses in a specialist cancer unit. In addition, individual interviews were conducted with patients who had used the OPNC service. Four themes became distinct through a synthesis of the material from the interviews and the messages: "a means for managing illness-related concerns at home," "a means for ensuring information flow," "a means for strategic information seeking," and "not yet available when needed most." Individualized information provided by nurses with access to their medical record was shown to be important to these patients. The findings of this study indicate that not only may access to an OPNC service help patients fulfill their otherwise unmet information needs, but also it may prevent delays and discontinuity in care due to informational gaps and lead to improved patient safety.
